バースレビューの論文 review on birthreview

産後ファミリー外来を開始するに当たり、バースレビュー「どんなお産でしたか？」とdebriefingを行なうことについて検証。

読んだ論文（skim readingですが）

分かったこと
正常なお産についてはデータがない。
traumatic birth, operative birth, instrumental birthなどについては、RCTが存在するが、結果は差がないか、若干悪化する可能性を示唆。
多くは助産師によるdebriefing
助産師は自由診療としてdebriefingを提供している所もある。

うちではどうするか
データが存在する、介入の強かったお産はおそらくこないと思われる。正常なお産については、positive reinforcementを期待して、ごく簡単でinformalなbrith reviewをおこなう。
一定期間後見直し。

いわゆる時間を確保したレクチャーではレジデントの知識保持は短期的には10ポイント高いが、長期的（1週間から6ヶ月）には差が無い。講義よりももっと、臨床経験を最大化するほうが学習効果が高いのではないか。
ただしレジデント3年目のみ、短期スコアと長期スコアに有意差がなく、長期的に記憶を保持できる。（臨床経験があるために有機的に結び付けて記憶しやすいからか）

ＡＡＦＰ，ＡＡＰ，ＡＣＰ，ＡＯＡがpatient centered medical homeについてjoint principle statementを提示（２００７）
そもそもＡＡＰ(米国小児科学会）が1967年に、カルテを1箇所にという概念で提唱、2002年により広い概念でのmedical homeを提唱。2004年には米国家庭医療学会が、personal medical homeの概念をＡＡＰの承認とともに提唱、ＡＣＰはadvanced medical homeという概念を2006年に提唱の経緯で、今回の発表にいたる。内科、小児科、家庭医が同じ方向を向いて、何かに合意するというのは非常に重要なこと。日本も見習いたい。必読。
